Application form requires some section to be filled with black ball point pen and other section by HB pencil only. Fill all the ovals and the squares with HB pencil only. Darken the oval and fill the letter/ number in the square mentioned against the same. Fill the Address, Signature, Details of parents / guardian with Black Ball Point Pen only.
1. Name of the Applicant (in CAPITAL Letters):
Write your name in capital letters as mentioned in your original 10th Pass or equivalent Certificate. Leave a space between each part of your name and then darken the appropriate oval given below the name. if your full name, exceeds 30 characters and does not fit in the space provided, shorten your name by using initials. Leave a blank space between initials. Do not use any titles like Mr., Mrs., Dr., and Miss/Ms. etc. e.g.: if your full name is KONIDELA SHIVA SHANKAR VARAPRASAD shorten it as, K S S VARAPRASAD.
2. Date of Birth: Write your date of birth as recorded in the 10th Pass Certificate/Birth Certificate issued by the competent authority in the format provided and then darken the appropriate oval provided below it. e.g.: if your date of birth is 3rd September 1987 fill as 03/09/87
3. Gender : Write the Code in the square and darken the appropriate oval.
4. Residence: Write the Code in the square and darken the appropriate oval for residence you belong to.
5. Nationality: Write the Code in the square and darken the appropriate oval.
6. Religion: Write the Code in the square and darken the appropriate oval.
7. Domicile (Please enclose proof): Write the Number in the square and darken the appropriate oval for the domicile which you belong to. Candidates arerequired to mention the Domicile to which they belong in the Application Form along with the attested photo copy of the domicile certificate.
8. Category (Please enclose proof): Write the Number in the square and darken the appropriate oval for the category you belong to. Candidates are required to fill all the reserved categories to which they belong in the application form along with the attested photo copies of the certificates as proof. Candidates belonging to NRI Sponsored category are required to download the Sponsorship Form from the CLAT website and duly filled in form should be attached along with the Application Form.
9. For NRI/NRI Sponsored Applicants:
1. Only NLIU, Bhopal; WBNUJS, Kolkota and HNLU, Raipur are offering seats for NRI/NRI sponsored through CLAT-2010.
2. Applicants for NRI/NRI sponsored category will not be considered for general or other category seats under any circumstances.
3. Applicants are advised to refer to the web sites of concerned universities for number of seats, definition and eligibility rules of NRI/NRI sponsored candidates and fee schedule.
10. NRI/NRI Sponsored Applicant are advised to give preferences for the University offering the seats in this category.
11. Qualifying Exam: Candidates who have passed the qualifying examination have to darken the appropriate oval. Candidates appearing / appeared whose
result has not been declared for the qualifying examination in the year 2010 have to darken the appropriate oval of the examination they have appeared.
12. Year of Passing and 13. Percentage of marks: Candidates who have passed the qualifying Examination should enter the percentage of marks secured in the qualifying examination and the year of passing and darken the appropriate ovals provided for. Candidate appearing for the qualifying examination or awaiting result should leave these columns blank.

Provide your address for correspondence. All formal communication from CLAT will be sent to you at this address only. Use blank spaces to separate words. Specify the name of your City. State and Pin Code in the columns provided. Also provide the mobile phone for SMS, fax and email ids. e.g.: if your address for communication is:
#32, H.F. Colony, 5th Cross, 3rd Main Road SP Nagar, 2nd Phase HAL. Use three lines given to fill up the information appropriately.

14. Test Centre Code: Write the Code of the Test Centre in the square provided. The applicant must indicate Three Test Centre options as per your preferenceand then darken the appropriate oval provided below. List of Test Centres and Codes are also given below:

CLAT reserves the right to allot any test Centre out of the three indicated by the candidate. No request for change of Test centre will be entertained in any circumstances.
15. Annual income of parents/ Guardian: Write the Number in the square and darken the appropriate oval for the total annual income of your parents/ guardian.
16. Preferences for Universities for admission: Write the preference of the University in the square and also darken the appropriate oval to indicate the order of preference for admission at different National Law Schools. Candidates are required to indicate their preference of the Law Schools in which they wish to join in order of preference for all eleven National Law University in the Application From.
Application with all the enclosures should be put straight (Do not fold) in the envelop given with the form and sent through Registered Post/Speed Post/Courier, so as to reach on or before 02nd April, 2010, by 5.00 P.M.:
